Pelvic Pain Treatment in Surrey

Expert care from Mr Sumit Kar, FRCOG, MRCOG, MD (Obs & Gynae), MBBS

Chronic pelvic pain is a common condition affecting many women, with pain lasting for six months or more. The causes can be varied and complex, often requiring specialist investigation to identify the underlying reason and determine the most effective treatment approach. As a Consultant Gynaecologist with over 25 years of experience and a Diploma in Gynaecological Endoscopy, Mr Sumit Kar offers comprehensive evaluation and management of pelvic pain. His expertise in diagnostic and operative laparoscopy allows for accurate diagnosis and treatment of conditions that may not be visible on routine scans.

  • 3Transvaginal ultrasound scan to assess the uterus, ovaries, and pelvic structures
  • 4Blood tests to rule out infection and other causes
  • 5MRI scan for detailed evaluation of deep endometriosis or complex pathology

Treatment Options

Medical Management

Pain relief medications including anti-inflammatories and hormone treatments may help manage symptoms. The contraceptive pill, progestogens, or GnRH analogues can be effective for hormone-related pain such as endometriosis.

Diagnostic Laparoscopy

A keyhole surgical procedure to directly visualise the pelvic organs and identify conditions such as endometriosis, adhesions, or other abnormalities. This is often the only way to definitively diagnose certain causes of pelvic pain.

Laparoscopic Treatment of Endometriosis

When endometriosis is found during laparoscopy, it can often be treated during the same procedure. This may involve excision (cutting out) or ablation (destroying) the endometriotic tissue to relieve symptoms.

Adhesiolysis

Laparoscopic division of adhesions (scar tissue) that can cause organs to stick together and result in pain. Adhesions may form after previous surgery, infection, or from conditions like endometriosis.

Treatment of Ovarian Cysts

Ovarian cysts causing pelvic pain can be removed laparoscopically while preserving healthy ovarian tissue. This includes endometriomas (chocolate cysts) commonly associated with endometriosis.

Multidisciplinary Approach

For complex cases, a multidisciplinary approach may involve pain specialists, physiotherapists, and other healthcare professionals to address all aspects of chronic pelvic pain.

What to Expect

During your initial consultation, Mr Kar will take a comprehensive history of your pain, including when it started, what makes it better or worse, and how it affects your daily life. A pelvic examination and ultrasound scan are typically performed to assess the uterus, ovaries, and surrounding structures. Based on the initial assessment, Mr Kar will discuss the most likely causes of your pain and recommend a management plan. This may include medication, further investigations such as MRI, or diagnostic laparoscopy if the cause is not clear from other tests. If laparoscopy is recommended, Mr Kar will explain the procedure in detail, including the potential to treat any abnormalities found during the surgery. His extensive experience in minimally invasive surgery ensures the procedure is performed safely with optimal outcomes. A multidisciplinary approach may be suggested for complex cases, involving other specialists to ensure all aspects of your pain are addressed. Treatment decisions are always made in partnership with you, taking into account your individual circumstances and preferences.

Frequently Asked Questions

Common causes include endometriosis, ovarian cysts, adhesions (scar tissue), fibroids, adenomyosis, pelvic inflammatory disease, and sometimes interstitial cystitis or irritable bowel syndrome. In some cases, multiple factors contribute to the pain. A thorough investigation is important to identify the specific cause.
Diagnostic laparoscopy is a minimally invasive keyhole surgery where a small camera is inserted through a tiny incision in the abdomen. This allows direct visualisation of the pelvic organs and can identify conditions like endometriosis or adhesions that don't show up on scans. If problems are found, they can often be treated during the same procedure.
Endometriosis is one of the most common causes of chronic pelvic pain. It occurs when tissue similar to the womb lining grows outside the uterus, often on the ovaries, fallopian tubes, or pelvic lining. This tissue responds to hormonal changes, causing inflammation, scarring, and pain, particularly during menstruation.
Not all pelvic pain requires surgery. Many women benefit from medical management alone. However, if scans and other tests don't reveal a cause, or if initial treatments aren't effective, diagnostic laparoscopy may be recommended to investigate further and potentially treat the underlying cause.
Most women can go home the same day as their laparoscopy. Recovery typically takes 1-2 weeks, during which you may experience some bloating and discomfort. You can usually return to normal activities within 1-2 weeks, though this depends on whether any additional treatment was performed.
Many causes of pelvic pain can be effectively treated, leading to significant improvement or resolution of symptoms. The outcome depends on the underlying cause. Conditions like endometriosis may require ongoing management, while pain from adhesions or cysts may be resolved with surgery.
